We are looking care coordinators to join the Coast team. The COAST (Croydon Outreach Assertive Assessment Support Team) is a multi-disciplinary specialist team which works with people with first episode psychosis in Croydon, providing a comprehensive Early Intervention Service.
We have expanded and looking for enthusiastic, motivated and highly energetic professionals to join us. The post holder should be a qualified nurse, occupational therapist or social worker and will undertake the responsibilities of care coordinator as part of the Care Programme Approach for a defined caseload of service users and their family members or carers. The post holder must be able to work as part of a team and on his/her own initiative, participate on the duty rota and formulate flexible, robust care plans tailored to each individual.
The Team supports over 300 service users across the London Borough of Croydon, we are based at Queen's Resource Centre in West Croydon. The service aims to positively focus on recovery with adults experiencing their first episode of psychosis, working with them to install hope and supporting them to maximise their potential.
There will be opportunities to be involved in groups (e.g. carers, social inclusion, psychoeducational), physical health clinics, family interventions and service development and research.The successful candidate will have an optimistic, recovery-focused approach and be able to demonstrate a specific interest in working with this client group.
It is an exciting time to be working in the Early Intervention. This post will offer excellent opportunities for qualified professionals looking to develop their skills in this area, facilitated by a highly motivated and supportive team, specialist supervision and opportunities for further training (e.g. Family Interventions, Phlebotomy, EI induction training). As part of your professional development, you will be expected to lead and supervise students. You will also be expected to actively participate in your own supervision and appraisal.
