The Rapid Response Team operates 24 hours a day, 7 days a week to avoid admission to hospital, facilitate discharge and to provide unscheduled patient care.
We have a vacancy for a Health Care Assistant who is looking to develop their skill base within the Community. This vacancy is at our Tonbridge base but we actively promote cross site working between here & our base at Coxheath.
You will be working alongside registered nursing staff delivering care to people in their own homes. You need to be a good communicator, have a caring and compassionate nature, work well as part of a team and be calm in a crisis. Experience of working with older people would be beneficial.
Good communication skills both written and verbal are essential. You will be required to maintain a working relationship with a range of other professionals within the Multi Disciplinary Team.
Good IT skills are essential. There is a wide variety of training available and protected time within working hours is given to attend.
As you will be visiting patients in their own homes you must be able to travel around the Trust in a timely manner.

You will develop new skills and knowledge whilst working with an established team, gaining valuable work experience. This is a full time vacancy but part time hours may be considered.
