We are looking for an experienced and motivated full-time grade 10/11 social worker to join our busy dynamic hospital team. The role is based at Salisbury District hospital however you will be part of the East hospitals SW Team. The role is a fairly new position. The role entails a lot of working independently/remotely at SDH whilst having support from a team based in Royal Bournemouth Hospital. Part of this role will be for you to support with the development of MDT practice at SDH and in promoting good communication and joint working.

This is an exciting opportunity as we work towards continuous development of integrated working within health and social care services.

You will facilitate timely discharges and admission avoidance planning for our customers. Experience of working in an integrated hospital setting is preferable but applications from candidates with transferable skills, who are motivated to undertake a new challenge are encouraged. Within this exciting opportunity you will be responsible for managing a range of complex cases such as complex Best Interest Assessments, Deprivation of Liberty and Court of Protection work, encompassing complex risk assessment.

You must have a degree in Social Work / Occupational Therapy or equivalent, be registered with the HCPC and be able to fulfil the travel requirements of the post.
