This is an exciting opportunity for a Rehabilitation and Reablement Assistant to work in the Community Independence Service (CIS) where you will deliver support to Portsmouth residents in order to restore, maximise or prevent deterioration in their physical, psychological and social wellbeing.
Helping people to lead independent healthy lives at home, to deliver Adult Social Care's purpose of: 'Help me, when need it, to live the life I want to live' through the delivery of flexible, holistic support plans.
The (CIS) is part of a co-located health and social care service which aims to work in an integrated way with health colleagues, designed to provide support to adults with health and care needs.
The CIS is currently undergoing further enhancements to the service, which will enable a more flexible and integrated approach to service delivery.
We are ambitious in our plans to deliver innovative and effective services that put the customer at the centre of what we do, working in more flexible ways to understand how we can put the right support in the right place at the right time to help people live the lives they want to live.By doing this, our aim is to improve the quality of care provided, reduce demand for social care services, reduce acute admissions (and therefore increased demand for ASC services) and length of time spent in hospital, as well as reducing dependency on long term care.
You will receive appropriate training and will achieve competencies leading to the Care Certificate. There may also be opportunities to attain a formal NVQ qualification in Health and Social Care.
